Complete the details
Select the assignee. Use 'Assign to me' if you are going to carry out the corrective action yourself. Otherwise click the 'Select' option to look up the list of eligible employees for assignee role. Then choose a Due Date for this CAR
If you are logging this CAR on behalf of someone else, you can change the author.
Upload any necessary documents to support the CAR information using the 'Attach Document' option
Remember to give each attachment a name and/or URL, otherwise the image will not be saved.
Once you make the attachment, you will be able to see it in the Image Preview tab
Once you either Save or Assign an extra section is displayed for Related Events. You can link any associated record from an existing CAR or a record from another module such as an audit record. Use 'Attach Entry. If you just Assigned the CAR then the Assignee will see this and not the author.
Choose the related event(s) by putting a check mark against those you wish to add and choose the 'Select' option in the bottom right corner of the page
Now choose 'Assign' which will save and send to the chosen assignee. Save and Exit if you need to return to edit prior to assigning. Assigning will move the CAR to the next step, Root Cause and Implementation. See separate article on Root Cause and Implementation.

Are there any standard guidelines or rules we can refer to for determining the levels of Severity, Occurrence, and Detection (SOD)?

It seems there are not enough departments to select from. Currently, we only have seven departments listed.
