Assign Team Members for the Fishbone or any of the other Root Cause Analysis Tools by using the Attach Entry option to look up names from the directory.
Click on Add Entry to create a Root Cause type
Select to pick an event.
Select for example the document which was the source for this Corrective Action Request. This may be an audit for example.
Start creating the Fishbone causes by choosing 'Add Entry'.
You can continue to add questions and answers to each step and causal factor as required until root cause(s) are identified. Click on the + against each question/answer to add commentary. This allows you to branch off if for example, multiple causal factors are identified.
Enter the category and the cause and click 'Save and Add Entry'. Signify a root cause by putting a check mark in the box.
You can create as many causes as necessary and use the + button to add more.
Click on the Refresh button to generate the Fishbone.
Once the Fishbone and any other analysis is complete you must now create entries for the identified Root Causes at all levels as identified. Click on 'Add Entry' and fill out the necessary boxes.
At least one Action Plan is mandatory to complete any CAR. Select Add Entry to create an action plan to correct any root cause finding. You will not be able to send a CAR for approval without an action plan being created and completed.
Complete the details
Click on Execute Action Plan
Before sending for approval, consider logging any lessons learned.
Consider the cost of quality for the changes made as well as the cost of implementing the corrective action
Once all sections are completed as required and consequential actions are created you can select' Approve' to move to the Verification of Effectiveness step. The Root cause may be rejected in which case a reason will be given and the action reverts back to the assignee to take further action.
Before final closure, the Effectiveness Review must be completed. Click on Add Entry.
Complete the effectiveness review log. You can also create a next review date if required.

Click on Close
