From the Search Apps, click to reveal the available modules and select Corrective Action Reporting
Select Add Entry
Choose 8D
Complete the necessary details, using the 'Select' option to access required records previously loaded.
Select 'Save' if you are need to wait before assigning. Otherwise select Assign. This will generate a reference number and send the 8D to the assignee.
It now moves from Draft to Root Cause phase
Select all necessary team members by using the Attach Entry option and searching the directory for those who need to be on the 8D team.
In D2: Problem Definition, start by choosing the Severity, Occurrence and Detection scores to obtain an RPN for this incident. This can also be obtained directly from the corresponding FMEA
Click on the tab for D3: Investigation and Correction step. You can Create Action Plans for each issue that needs investigation. This can be a local or Global issue. see Global Action Plan.
Complete the necessary detail for the action plan and Execute
When all the details of each action plan are completed you can Complete. Comments can be added at any time during or after creation of the Action Plan.
Specify at this stage if review of the actions taken is required or not.
If a review is required then this will need to be completed before moving forward
Once all is completed you can Approve and Close. You can also create another action using the More Action Required option.
Each action item you create will be attached to this section.
At this point it is advisable to start building the Cost of Quality section to track all associated costs. This can be edited at all stages of the investigation.
Related events is also a good section to consider from the start as all related outputs of the 8D can be linked for reference.
You should also use the Document Attachment section to start attaching related files.
You can click on the D4 step at any time to start completing root causes
For creation of a 5 Why, Fishbone or any other action available from this section, see other available articles describing how to generate these.
As part of the D5 step and as a result of your 5 Why analysis for example, you need now to create action plan(s) to address the findings of the analysis. These can be local or global in nature.
You can Save as Draft or Execute each action plan when all details are complete
You can choose to have a review of the actions by creator or supervisor.
Once each action plan is completed you can Complete.
Click on the D6: Implementing and Validating tab to create action plans for this step.
The action plans for D6 can again be local or global in nature.
Execute the plan once all details are entered and ensure completion notes are made before moving on.
As with previous steps a review by creator or supervisor can be selected as well as not required option
When complete you can Approve and close and choose more actions if required.
Select Approve to move to the Verification of Effectiveness step
Add New Lesson
Complete the details
Choose Continue to move to the next Lessons Learned step of entries to detail what has been discovered in this 8D and notifying individuals.
You now need to complete an Effectiveness Review to review the entire 8D process and all related events prior to closure.
The Effectiveness Review Log will create an additional section similar to the above
Create a list of all individuals who need to be included in the closure notification
Make sure that the Cost of Quality section is updated.
Once the 8D is completed you can Close.
